Employee-paid Long-Term Buy-Up and Short-Term Disability Insurance Plans – Not all employees are eligible for these plans. Only New Hires, Rehires, and employees who experience an employee status change that makes them newly eligible for the employer-provided “Long-Term Disability Core plan” (Benefit Plans Handbook Page 6-9) are eligible for enrollment in these plans without a Health Assessment, except as noted in the handbook and information provided on the below links. Eligibility is for employees with a work schedule of 60 or more hours per pay period except where noted:
